Matrix Home Health Care Specialists has an opening for Home Health Aides to care for our in-home clients.
We are currently in need of weekend evening (3:00pm-11:00pm) and overnight (11:00pm-7:00am) caregivers for a St Louis Park client, caregivers for short shifts on Sunday ( 10:00am - 3:00pm), Monday (8:30am - 12:00pm and 4:30pm - 8:30 pm) and Tuesday (8:30am - 12:00pm), and caregivers for overnight (8:00pm - 8:00am) shifts with clients in Richfield and Minneapolis.
This position reports to the Director of Nursing or an RN Supervisor.

Description: A home health aide or homemaker/ home health aide or personal care assistant, is responsible for providing service to an ill or disabled individual in the absence or disability of caregiver; or if caregiver is in need of personal time to maintain ongoing level of care provided, this includes household management techniques for self care and social support, home safety practices and housekeeping needs.

JOB DUTIES / RESPONSIBILITIES:

  1. Assist in a program of rehabilitative nursing to help each client reach and maintain their highest level of performance and independence.
  2. Perform normal household services essential to health care at home including shopping, laundry light housekeeping, and meal preparation. Have
  3. Knowledge of nutrition and food preparation and be able to provide adequate planning and preparation of meals to promote and maintain level of nutritional status.
  4. Assist the client with personal care activities including bathing, skin care, back rub, hair care, nail care, dressing and undressing, feeding, oral hygiene, shaving, grooming, and bedtime care.
  5. Does treatments if properly trained including TPR, weight, hot and cold applications, application of support stockings and ace bandages, wound isolation techniques, decubiti care and diabetic foot soaks.
  6. Assist client to bathroom or in use of urinal or bedpan. Keeps incontinent clients clean and dry.
  7. Give catheter care, collect urine and stool specimens, do diabetic urine testing, give suppositories and enemas if properly trained.
  8. May record intake and output
  9. Assist the client with exercise, ROM, ambulating, transfer activities, may be under the supervision of physical therapy of occupational therapy, is aware of good body mechanics.
  10. Observe client, reporting and changes in physical condition, attitude, reaction, appetite, etc. to nursing supervisor.
  11. Assist self-administration of medications by client.
  12. Participate in care conferences with other members of the health care team.
  13. Keep confidential any knowledge of clients condition and personal problems.
  14. Document as indicated in the clinical record.
  15. Possess an understanding of human behavior. Provide appropriate level and kind of social and emotional support to the client/caregivers enabling them to express concerns, etc. and maximize the clients ability to function in his/her home. Interact with client.
  16. Assist with personal communication skills as needed.
  17. Know and practice good safety habits
  18. Perform additional job duties as assigned by RN supervisor.
  19. Dont spend excessive time on personal phone calls. Arrive on time.
  20. Send required documents to the office in a timely manner. (example: Progress notes, Time Sheets.)
  21. Attend inservice education as required. (12 hours per year.)
  22. Maintain communication with the office staff through reading newsletters, returning phone calls in a timely manner, updating availability as needed.

SAFETY PERFORMANCE DUTIES:

  1. Uses transfer belt whenever transferring and /or ambulating clients.
  2. Reports unsafe client behavior to RN supervisor (e.g. unauthorized smoking, combative behavior) so remedial action can be taken.
  3. Uses mechanical lifts (hoyer, century) whenever appropriate for transfers.
  4. Reports any defective equipment or environmental hazards to RN supervisor.
  5. Wipe up wet bathroom floors, spills and other falling hazards immediately.
  6. Observes special precautions as noted on the Home Plan of care for each individual client.
  7. Understands and uses Universal Precautions and necessary infection control measures at all times.
  8. States knowledge of all Emergency Procedures.
  9. Reports all incidents/accidents to RN Supervisor.

. Requirements: Educational Requirements:
High school graduate (or equivalent), CNA certificate preferred
Successful completion of the Matrix competency evaluation for home health aides.

  1. Must be able to read and write, comprehend and carry out directions and instructions.
  2. Able to function effectively with minimal direct supervision.
  3. The ability to accept the responsibilities and demands of the job.
  4. Good health status and emotional stability.
  5. Ability to perform essential functions and physical requirements with or without reasonable accommodations.

Working Conditions:
The Home Health Aide or Personal Care Assistance must have a genuine concern for clients of all ages. He /she must be capable of standing, walking, stretching, turning, stooping, bending and lifting as listed under working condition requirements. He/she must also be capable of finger and hand dexterity and visual and aural activity to detect changes in clients condition.
Working Condition Requirements:
1. In an 8-hour day, employee must be able to:
a) Sit for 1 hour
b) Stand for 3 hours
c) Walk for 4 hours
2. Employee job requires:
a) Bend/stoop frequently
b) Squat frequently
c) Crawl occasionally
d) Reaching above shoulder level occasionally
e) Crouching frequently
f) Kneeling occasionally
g) Pushing/pulling frequently
3. Employees job requires carry of:

a) Up to 10 lbs frequently
b) 11- 24 lbs occasionally
c) 24-80 lbs Very seldom
4. Employees job requires lifting of:
a) Up to 10 lbs frequently
b) 11-24 lbs occasionally
c) 24-80 very seldom
5. Job requires employee to be able to use hands for repetitive motion for simple, firm and fine manipulating.
